What is a Geriatric Care Manager?

Geriatric Care Managers can help reduce family stress and worry by ensuring that their loved one is maximizing resources and has an advocate to increase their quality of life.  We are available to respond to a crisis and navigate the healthcare system, handle the difficult issues that often arise, address the  immediate problem(s), and  remain connected once the crisis passes, if desired.

 Services provided by a Geriatric Care Manager may include:

• A complete assessment and evaluation of mental, physical and financial needs with support and care recommendations.
• Assistance finding appropriate housing and help with the transition to the new living environment such as an independent, assisted living facility or memory care facility if desired.
• Identifying and connecting with needed community services and resources.
• Act as a liaison for families that live out of the area by keeping them informed as I continue to monitor the senior's care and handle the coordination of additional services that may become necessary.
• Arrange and monitor services needed such as home care, home healthcare, palliative and hospice care.
• Making referrals to financial, legal or medical professionals.
• Assisting in the creation and sharing of Advance Directives and other life care and end of life plans.
