What is a Geriatric Care Manager?

Geriatric Care Managers can help reduce family stress and worry by ensuring that their loved one is maximizing resources and has an advocate to increase their quality of life.  We are available to respond to a crisis and navigate the healthcare system, handle the difficult issues that often arise, address the  immediate problem(s), and  remain connected once the crisis passes, if desired.

 Services provided by a Geriatric Care Manager may include:

• Assistance with medical care and concerns
• A complete assessment with care recommendations (usually completed in the senior's home)
• Assistance finding appropriate housing and help with the transition to the new living environment such as an assisted living facility or memory care facility
• Identifying needed community services and resources
• Act as a liaison for families that live out of the area by keeping them informed as I continue to monitor the senior's care and handle the coordination of additional services that may become necessary
• Arrange and monitor services needed such as home care, home healthcare, hospice
• If the senior remains living at home, weekly/bi-weekly home visits are made to ensure the senior is safe and well
