Sometimes families live far away and are unable to assist with coordinating and overseeing the care of a loved one. Other times, families may find that they are simply unable to provide the necessary care and assistance needed for someone they love as they face the transitions and challenges of aging and dying.
As a Geriatric Care Manager and End of Life Doula I can be a liaison between healthcare providers to ensure that your personal wishes and preferences are known and documented and that there is continuity of care so that you may live the way you choose, with dignity, until the moment of your death. Individuals should be allowed to maintain their self-esteem, independence and choices through the challenges of aging and dying.
I listen deeply and provide personalized, non-judgmental care to promote my clients peace of mind and sense of agency. I honor life and the mysterious enigma of death.
